Mary L. Polasik

Mary L. Polasik, 74, of Dunkirk died Wednesday, May 25, 2016 at Brooks Memorial Hospital.  She was born in Silver Creek, daughter of the late Dominic and Leona (Hatch) Damico.A retired employee of Dr. Zimmer in Dunkirk.

Her memberships include Blessed Mary Angela Parish Church of St. Hedwig.Surviving is her husband of over 55 years, Richard D. Polasik; son Rick Polasik and Kelly (Jack) Chlosta, both of Hamburg and granddaughter Jaylin Chlosta. Sister of Frances Jezioro of Angola, Myrta (Louis) Catalano of Angola, Dominic (Sharon) Damico of TN, her K9 granddog Hogan. Many nieces and nephews also survive.

Preceded in death in addition to her parents are a brother, Anthony Damico; brothers-in-law John Polasik and Edward Jezioro; and sister-in-law Helen "Pat" Damico.

A Christian burial service will be held Wednesday morning at 10:30 at the R. Mackowiak Funeral Home Inc., 56 East Doughty Street in Dunkirk. Burial will be in St. Hyacinth's Cemetery. Flowers are gratefully declined and Memorials can be made to Hospice of Chautauqua County, 20 W. Fairmount Ave., Lakewood, NY 14750.
