Jacqueline A. Formanowicz, 81, of Dunkirk, died, Monday morning, July 27, 2015, following a lengthy battle with Parkinson's Disease at Chautauqua Nursing and Rehab, in Dunkirk. She was born September 21, 1933, in Dunkirk, daughter of the late Eric and Virginia (Woelfle) Lancaster. Jacqueline retired from Dunkirk Public Schools after working as a lunch room aide at School #6 for 22 years. She was always fondly remembered as "Mrs. Forman" by the hundreds of School #6 students who passed thru its cafeteria doors. Her memberships include Blessed Mary Angela R.C. Parish, Church of St. Hedwig, the Kosciuszko Polish Home, and former School #6 Mothers Club. A devoted catholic, she always put her family first. She was also an avid reader and enjoyed musical productions in Buffalo and Toronto. Surviving is her husband of 63 years Daniel R. Formanowicz whom she married February 9, 1952 in Bristol, PA, their children Dr. Daniel (Kendra) Formanowicz of Orange, CA, Virginia "Ginger" (Laroy) Burgess of Henrietta, NY, Michael (Heidi) Formanowicz of Fredonia, daughter in law Risha Formanowicz of Dunkirk, grandchildren Eric (Sarah) Formanowicz, Nicholas, Stephen, Abigail, Megan Formanowicz, Elizabeth Burgess, Ann (Tom) Bersani, 2 great-grandchildren Corina and Samuel Bersani, sisters Virginia Wachter, Diane (Jeepers) Connelly, brother Paul (Chris) Lancaster, sister-in-law Rose Lancaster along with several nieces and nephews also survive. Preceded in death in addition to her parents are her son John who died in 2009 and brother David Lancaster, who died in 2013. Relatives and friends are welcome to call at the R. Mackowiak Funeral Home Inc., 56 East Doughty Street in Dunkirk on Friday from 3-7 pm. A mass of Christian Burial will be celebrated in Holy Trinity R.C. Church on Saturday morning at 10 o'clock.
